I understand that I am comitting to the TUMBLEBUS ongoing program and I can withdraw at any time as long as I contact the TUMBLEBUS office. I have read and agree to the policies of the TUMBLEBUS. I understand that my child will be taken to class each week unless I notify the TUMBLEBUS office. I understand that if my child is absent, I am expected to pay for that class. I understand that my child's picture may be used in promotional materials. No names will be used. I recognize a risk is involved in participation in gymnastics and related activities and that requires adherence to rules and discipline.
By checking this box, I, the parent/guardian, release TUMBLEBUS, their officers, instructors, and the daycare center of all responsibilities and all claims for injuries received while participating in TUMBLEBUS activities or practing gymnastics and its related activities.
